This agreement is entered into by , herein referred to as 'CLIENT(S)' and THE BELLO GROUP, INC DBA CHIOMA, herein referred to as 'TBGI'.

I agree and consent that the completion of this form, whenever booking Services via online (or through mobile app), purchase of Services or use of the Services constitutes an authorized signature, acceptance and agreement to the terms and conditions of this Waiver as if actually signed in writing. Further, I agree that no certification authority or other third party verification is necessary to the validity of electronic signature on this agreement. I agree that the lack of such certification or third party verification will not in any way affect the enforceability of the electronic signatures on this agreement or any resulting contract between myself and the TBGI.

Additional Agreement of Client:
Client agrees that all treatments purchased must be used within 12 months of purchase. Unused treatments will expire after 12 months. Monetary refunds are not issued for unused services. In the event that a client cannot continue with treatments, all unused services are non refundable.
Client agrees that TBGI has substantially discounted the normal fee per treatment as an inducement for clients to pre-pay for this package of treatments enumerated above. Therefore, if clients were to purchase these treatments outside of this agreement, the fee would be at FULL RATE OF EACH SERVICE.
FORCE MAJEURE. Service Provider will not be liable for any delay or failure to perform its obligation under this Agreement if prevented from doing so by acts of God, acts of war or civil disturbance, pandemics, governmental restraints, utility or communications failures, or any other causes that the affected party could not, with reasonable care, control or prevent. If such an event occurs, then where practicable, the affected party will use reasonable efforts to promptly resume its obligations under this Agreement upon cessation of the Force Majeure event.
Clients agree that these esthetic treatments and by their very nature subjective in results and satisfaction; and affected by a wide variety of factors such client's lifestyle, genetics which is beyond TBGI's control. Client understands that interrupting any recommended treatment schedule will also mitigate the end results. As such, client agrees that this agreement may not be CANCELLED and is NON-REFUNDABLE.
Client agrees that treatment rooms, equipment and staff are solely assigned to their treatment appointment and that any late arrivals after the 10 minute grace periods OR no-shows, missed, cancelled appointments without a minimum of 48 hours notice, will be subject to a cancellation fee of $50.00. Client cannot be seen or rescheduled until the $50 fee has been paid. If there are additional treatments left, Clients may choose to forfeit the missed session and complete remaining sessions.
Client agrees that all complementary, voucher and upgraded treatments are added at the end of the contractual treatments and, as such, have no refundable value.
With respect to, Package Fee Schedule, Payment Schedule, Treatment Schedule, Refund Policy and Appointment Cancellation Policy, this Agreement supersedes any and all prior agreements and arrangements or understandings between the parties relating to the subject matter hereof. No oral understanding, statements, promises or inducements contrary to the terms of this Agreement exist. This Agreement cannot be changed or terminated orally.
